Example: 

-You are a member of ASSF in your 2nd year and 3rd year and you are currently the president in your 4th year. So write it as ----- ASSF [2,3, Pangulo 4]

-You are a member of ASSF in your 2nd year and 3rd year and you are currently the president in your 4th year plus you are a member of the NSEC in your 2nd year and the Program Committee Head in your 3rd year, add a semicolon (;) in between the clubs. 

So write it as ------ ASSF [2,3, Pangulo 4]; NSEC [2, Program Head 4]

-If you are also the division representative of the College of Nursing, 

So write it as -----
